Medical Records

To initiate a request for medical records from Shaw Cancer Center, call (970) 569-7617 or download, print, complete and sign the following form.

• Authorization to Release Protected Health Information (pdf)
• Autorización para divulgar información médica protegida (pdf)

   

Instructions: Please complete ALL portions of this authorization and bring a printed copy with you to the front desk at Shaw Cancer Center or call (970) 569-7617. The form may also be faxed to (970) 470-6403. Please specify if you would like to pick up the copies in person, send them to another facility, or have us mail them to you. 

Remember - you can access your medical information through the Vail Health patient portal.
 

Organizations

To initiate a request for medical records from Shaw Cancer Center, please fax your request on office letterhead with the patient’s name, date of birth, dates of service, and types of records you would need. If requesting for multiple patients, please send separate requests.

Note: Requests for medical records are processed in the order they are received. Medical records will be delivered within 10 days upon receipt of request.

Mailing Your Medical Records to Shaw

If you are a patient and need to have your outside records sent to Shaw Cancer Center, please note that we do not receive mail from USPS to our physical address—FEDEX/UPS only to Shaw Cancer Center.

How Long Does Vail Health Retain My Radiology Images? 

Vail Health will only retain radiology images for ten (10) years past the date the images are taken or ten (10) years past the age of eighteen (18) for minor patients.
