Shaw at Home, a program developed as an extension of Vail Health Shaw Cancer Center, is an in-home, community-based palliative care program that supports the needs of Shaw cancer patients and their families in the Eagle River Valley. This program seeks to improve patient access, affordability and continuity of care from a care team they have come to trust. The support and intervention provided through Shaw at Home improves the patient experience, reduces suffering, lowers health care costs and enhances treatment and outcomes.
Through access to an interdisciplinary team of supportive care providers and resources, our goal is to ensure all Shaw patients receive whole-person care throughout their cancer journey.
Shaw at Home
Shaw at Home is a community-based palliative care program that serves patients and families in their homes, where healing transpires best.
Palliative care can be provided while a patient receives treatment for cancer. Palliative care is offered at time of diagnosis, regardless of prognosis, and seeks to reduce symptoms, distress and hospitalizations while improving quality of life. Hospice is offered when a patient is known to be approaching end of life, typically within six months, and seeks to provide comfort during the dying process.
Shaw at Home’s palliative services reduce symptoms, distress and hospitalizations while increasing quality of life during the experience of a cancer diagnosis.
All Shaw Cancer Center patients are eligible to participate in palliative care services. In-home palliative care services are currently available to patients living in the Eagle River Valley of Eagle County. All patients have access to palliative services via tele-health for enhanced symptom management.
Shaw at Home palliative care services are provided by a specialized team of nurse practitioners, nurses, social workers, spiritual care providers and volunteers that work with the Shaw oncology team to promote healing and reduce distress at any point during the cancer journey, from diagnosis to cure or transition of care. Shaw at Home reaches patients in their homes, while treatments and appointments with the oncology team remain in clinic.
No. Shaw at Home will not infuse chemotherapy, immunotherapy or other drug infusions within the home setting. What you can expect is that Shaw at Home team members can provide home hydration, lab draws, wound care management, symptom management, education, counseling and other supportive services.
No. This program is for any Shaw Cancer Center oncology patient experiencing emotional or physical distress or anyone interested in formulating a plan to reduce emotional or physical distress before a situation becomes a crisis. Oncology patients are eligible for Shaw at Home regardless of prognosis.
No, you will still see your Shaw oncology team as you usually do. Your Shaw oncology team and Shaw at Home Team work together to provide an individualized care plan based on your needs, values and goals.
If you are a current patient with a primary oncologist at Shaw and are experiencing physical or emotional distress related to your diagnosis, we are here to help. You do not need to wait until you are in crisis to access our services.
Most insurance carriers cover palliative care, and the Shaw at Home team will work closely with your insurance carrier to verify coverage and benefits. If you are not insured, or if you have concerns about financial eligibility for Shaw at Home, our palliative care social workers can help find resources so you receive the help you need.
